Florida Document Requirements

The suggestions listed below are based on the February 2019 revision of the Florida MVR Checklist. View current Florida statutes

Shop Information

Motor Vehicle Business Registration Number

Fill in the following: 

  • Name - Florida
  • Number - MV{fill in your license number}

Note: The information entered here will display at the bottom of printouts.

Print Settings

Document Options

Choose to Show the following on the Printed (or emailed) Document:

  • Proposed Completion Date - Appears in the Header
  • Save Parts for Inspection - Appears in the Footer
  • Intended Method of Payment - Appears above the Customer Signature line
  • Expiration - You can set an expiration date that prints on the estimate and shows the number of days for which the Estimate is valid. This information is only used for content display purposes on the customer printout, so that your customer will know when the printed estimate expires.

Document Header & Footer

  • Shop Warranty and Disclaimer - Appears in the Footer section next to the Totals. Maximum Characters: 1000. Copy and paste the following text:
    • 1 YR/12,000 mile limited warranty on all parts and labor. Customer will be charged $45.00/DAY for vehicles not picked up within 3 days of completion. Labor is based on hourly rate only.  All parts new unless otherwise stated.
      • Notes about the text above:
        • You must modify warranty information, charges, fees and any other disclaimers as it applies to your shop. The information used here is for example purposes only.
        • You must state if your shop calculates labor based on hourly, flat rate or both.
  • Estimate Footer Text​ - Appearance can be customized in the editor and appears at the bottom of each page of the Estimate. Maximum Characters: 1000. Copy and paste the following text:
    • *Shop Supply & Hazmat Fees: This charge represents costs and profits to the motor vehicle repair facility for miscellaneous shop supplies or waste disposal. ***FS403.718 mandates a $1.00 fee for each new tire sold in the State of Florida. ***FS403.7185 mandates a $1.50 fee for each new or remanufactured battery sold in the State of Florida. The Estimate and Diagnostic Fee is based on an hourly rate of $90.

      • Notes about the text above:
        • The section in red must be set to 12pt font in the text editor.
        • You must display the charges for creating an estimate. If the charge cannot be predetermined, you must include the basis on which the charge will be calculated. 

 

On Each Document 

"Complete by" Date and Time

Select a date in the Proposed Completion Date field

Alternate Contact

If an alternate contact has been requested by the customer:

  1. Select the Additional Authorizer + button on the customer edit screen.

    • Last Name
    • First Name
    • Phone Number

Mileage In and Mileage Out

Enter Mileage In and Mileage Out values.

Work Descriptions

  • Add a Customer Concern Note:
    1. Select Note.

    3. Enter a general description of the customer's problem or request for repair work or service relating to the motor vehicle.

  • Add a Work Performed Note:

Select the part Description field to edit the text >> Enter a condition of the part (E.G., Used, Rebuilt, Reconditioned)
